We regret the inconvience; but, the application is no longer available as an executable file. Be sure to include a signed check with the application. The form is designed to either be mailed in a legal size envelop or folded and mailed. The post office prefers that items NOT be stapled. If you use tape, please try to seal the tape fully to the paper. It would probably work best if mailed in a standard envelope. Postage IS required.
